Family Satisfaction Survey: Last chance to participate

This the last opportunity to submit a list of family contacts for inclusion in the 2021 Family Satisfaction Survey. All family lists must be submitted by February 15, 2022.  Facilities that do not provide a list of family contacts will be listed as "Did not participate" on the Ohio Long-Term Care Consumer Guide website.

Scripps Gerontology Center has created a report of the submission status of every Nursing Home and Residential Care Facility as of February 2, 2022. Those lists are available at 2021 Family Satisfaction Survey and Long-Term Care Guide Consumer Guide website under “Resources for Facilities."  

Please review the list to confirm whether or not your facility has submitted.  Even if you think you've submitted a list, please double-check the list because some submissions have required further action in order to be processed and Scripps has been trying to reach facilities to resolve these issues. These facilities are found in the "Other" column.

If your facility is listed as "Not Submitted" or "Other", please contact the Scripps Gerontology Center Family Survey Helpline at 1-844-864-0049 now for instructions on how to submit your family list or resolve any outstanding issues.
